In solid organ transplantation, time is the ultimate currency.
Every minute a deceased donor kidney, liver, heart, or lung spends outside the human body—known as cold ischemia time—progressively reduces post-transplant graft survival and increases delayed graft function ($\text{DGF}$). For decades, donor-recipient compatibility required performing physical cell-based Complement-Dependent Cytotoxicity ($\text{CDC}$) crossmatches, adding 4 to 6 hours of delays while live donor lymphocytes were isolated, incubated with recipient serum, and evaluated under microscopy.
Today, the transplant immunology community is undergoing a generational shift toward HLA Virtual Crossmatching ($\text{VXM}$).
OpenPHR’s newly published Cookbook 303 (Offline Clinical Transplant Immunology HLA Virtual Crossmatch & DSA MFI Engine) brings this complex immunogenetic pipeline into an open-source, containerized format compliant with American Society of Histocompatibility and Immunogenetics (ASHI) and American Society of Transplantation (AST) guidelines.
The Mechanics of Virtual Crossmatching
A virtual crossmatch compares the recipient’s circulating anti-HLA antibody repertoire—quantified via solid-phase Luminex Single Antigen Beads ($\text{SAB}$)—directly against the high-resolution molecular HLA genotype of the prospective donor across both Class I and Class II loci:
- Class I Loci: $\text{HLA-A}$, $\text{HLA-B}$, $\text{HLA-C}$ (Expressed on all nucleated cells; targets of cytotoxic $CD8^+$ T-cells).
- Class II Loci: $\text{HLA-DRB1}$, $\text{HLA-DRB3/4/5}$, $\text{HLA-DQB1}$, $\text{HLA-DQA1}$, $\text{HLA-DPB1}$, $\text{HLA-DPA1}$ (Targets of helper $CD4^+$ T-cells and key drivers of microvascular endothelial capillaritis).
Cookbook 303 mathematically compares the candidate’s anti-HLA IgG profile against donor-mismatched alleles, identifying specific Donor-Specific Antibodies ($\text{DSA}$) and calculating both dominant and cumulative MFI burdens:
\[\text{DSA}_{\text{total}} = \sum_{i=1}^{n} \text{MFI}_{\text{DSA}_i}\]Clinical Risk Stratification: When dominant DSA MFI exceeds $\ge 6,000$, or cumulative MFI exceeds $\ge 8,000$, the engine automatically flags a "High Immunologic Risk / Positive VXM" sentinel, preventing crossclamp until an aggressive desensitization protocol is initiated.
Automated Pre-Transplant Desensitization Protocol
For highly sensitized candidates ($\text{cPRA} \ge 80-95\%$), finding a completely crossmatch-negative donor can take years. Cookbook 303 generates complete pre-transplant desensitization orders:
- Therapeutic Plasma Exchange ($\text{TPE}$): 3 to 5 sessions ($1.0 - 1.5$ plasma volume exchanges) to clear circulating IgG antibodies.
- High-Dose IVIG ($2.0\text{ g/kg}$): Administered post-plasmapheresis for Fc-receptor blockade and anti-idiotypic neutralization.
- B-Cell Depletion: Rituximab $375\text{ mg/m}^2$ IV single dose.
- Lymphocyte-Depleting Induction: Antithymocyte Globulin (Thymoglobulin, $1.5\text{ mg/kg/day} \times 4-5\text{ doses}$, cumulative $6.0\text{ mg/kg}$).
OpenPHR View
By democratizing the software layer of histocompatibility and virtual crossmatching, OpenPHR is breaking down proprietary barriers in organ allocation, allowing regional transplant centers and OPOs to match organs faster, safer, and with complete algorithmic transparency.
