We maintain an expedited correction workflow for all licensed California child care facilities. If a citation listed on your profile has been successfully resolved, cleared by a CDSS licensing analyst, or closed with an approved Plan of Correction (LIC 9099):
How to submit official documentation:
- Send an email to corrections@openphr.org with your state license number (e.g.,
CDSS-013420891) in the subject line. - Attach a digital copy of your official CDSS Community Care Licensing Division clearance report (Form LIC 9099 / Plan of Correction sign-off) or updated state license certificate.
- Our data verification team reviews your state documents against the California public licensing docket within 3 business days. Once confirmed, your facility profile and Stability Index score will be updated immediately.
