1. Senior Home Care Levels • Plain-English Parkinson's Guide

Independent Living vs. Assisted Living vs. Memory Care vs. Skilled Nursing

When Parkinson's patients typically need each level of care—and what triggers a transition.

Independent Living (IL)

$2,800 – $5,200 / month

What it is: Private apartments or casitas in a senior community providing meals, weekly housekeeping, scheduled transportation, social programming, and campus maintenance—without licensed hands-on personal care or nursing administration.

When Parkinson's Patients Typically Need It

Early-stage Parkinson’s (Hoehn & Yahr Stage 1–2) when the person manages their own Carbidopa/Levodopa schedule independently, walks safely without human assistance, and wants to downsize home maintenance, stop driving at night, and access on-campus exercise classes.

Parkinson's Watchouts:
⚠️Independent living staff do NOT administer medications or provide physical transfer assistance during an "OFF" episode.
⚠️Families can often hire a private-duty home care aide or receive Medicare Part B outpatient PT/OT/Speech inside an independent living apartment as symptoms progress.
Medicare Room & Board: NOT Covered ($0)AZ ALTCS Medicaid: Not covered

Assisted Living (AL)

$4,200 – $7,800 / month (Phoenix Metro Median: $6,000 / month)

What it is: Licensed residential apartment community providing 24/7 caregiving staff, 3 daily meals, housekeeping, and structured assistance with Activities of Daily Living (ADLs: bathing, dressing, grooming, toileting, mobility transfers) plus licensed medication administration.

When Parkinson's Patients Typically Need It

Mid-to-advancing Parkinson’s (Hoehn & Yahr Stage 2.5–4) triggered by: (1) missing or mistiming multi-dose levodopa schedules at home, (2) recurrent falls or freezing of gait requiring standby transfer assistance, (3) orthostatic hypotension or nighttime bathroom falls, or (4) spouse/family caregiver burnout.

Parkinson's Watchouts:
⚠️Most assisted living communities price using a Base Apartment Rent PLUS a Care Level Tier (Tier 1 to Tier 4/5). As Parkinson’s motor fluctuations require two-person transfers or frequent bathroom assistance, the monthly care tier fee can rise by $1,000–$2,500/month.
⚠️Standard assisted living med-pass windows allow 60 minutes before or after a scheduled time—which is dangerous for Parkinson’s. Always verify in writing whether the community can deliver levodopa within ±15 minutes of prescribed clock times.
Medicare Room & Board: NOT Covered ($0)AZ ALTCS Medicaid: Covered (at ALTCS-contracted communities)

Memory Care (Secured Parkinson’s Dementia / Lewy Body Unit)

$6,200 – $8,800 / month (Phoenix Metro Median Estimate: ~$7,250 / month)

What it is: Specialized, secure residential neighborhood with higher caregiver-to-resident staffing ratios, locked/monitored exits to prevent wandering, structured low-stimulation daily routines, and staff trained in dementia behaviors and visual hallucinations.

When Parkinson's Patients Typically Need It

When Parkinson’s Disease Dementia (PDD) or Lewy body dementia features develop—specifically: (1) unsafe wandering or exit-seeking, (2) distressing visual hallucinations or delusions (Parkinson’s Disease Psychosis) especially in the evening ("sundowning"), (3) severe executive dysfunction where the resident attempts to stand/walk unassisted despite high fall risk, or (4) nighttime confusion reversing sleep-wake cycles.

Parkinson's Watchouts:
⚠️Do NOT move a cognitively intact Parkinson’s patient into Memory Care solely because of physical freezing or slow speech (bradyphrenia).
⚠️In Memory Care, ensure the medical director and consulting pharmacist strictly prohibit first- and second-generation dopamine-blocking antipsychotics (haloperidol, risperidone, olanzapine), which can cause fatal neuroleptic sensitivity or rigid immobility in Parkinson’s/Lewy body patients.
Medicare Room & Board: NOT Covered ($0)AZ ALTCS Medicaid: Covered (at ALTCS-contracted communities)

Skilled Nursing Facility (SNF) — Short-Term Rehab vs. Long-Term Care

$9,003 / month Semi-Private ($296/day) | $12,167 / month Private ($400/day)

What it is: 24-hour licensed medical facility with Registered Nurses (RNs), Licensed Practical Nurses (LPNs),Certified Nursing Assistants (CNAs), and on-site physical, occupational, and speech therapy.

When Parkinson's Patients Typically Need It

Two distinct scenarios: (1) Short-Term Post-Acute Rehab after a hip fracture, pneumonia, fall hospitalization, or DBS surgery complication to regain strength before returning home; or (2) Long-Term 24/7 Skilled Nursing (Hoehn & Yahr Stage 5) when the patient requires mechanical Hoyer lift transfers, complex PEG-J tube feeding / dysphagia management, stage 3–4 pressure ulcer care, or continuous skilled nursing supervision that exceeds an assisted living license.

Parkinson's Watchouts:
⚠️Post-acute SNFs frequently substitute hospital formulary schedules and delay Carbidopa/Levodopa doses. Bring the Parkinson’s Foundation Hospitalization & Rehab Kit and verify the exact admission MAR (Medication Administration Record) clock times on Day 1.
Medicare Room & Board: Short-term rehab only (up to 100 days after qualifying 3-day inpatient hospital stay); $0 for long-term custodial careAZ ALTCS Medicaid: Covered (at ALTCS-contracted communities)

Parkinson's-Specific Senior Living Evaluation Checklist (7 Critical Questions)

Standard senior-living tour sheets focus on dining menus and chandeliers. For Parkinson's, on-time levodopa delivery (±15 minutes) and freezing-of-gait training determine whether your family member stays safe or ends up in the ER.

0 / 7
Verified on Tour
#1 • Medication Timing (Critical #1)

Are Carbidopa/Levodopa doses administered ON SCHEDULE within ±15 minutes of the neurologist’s prescribed clock time—every dose, every shift?

Why it matters for Parkinson's:State regulations often permit assisted living and SNF med techs a ±60-minute "med-pass window." For a Parkinson’s patient taking levodopa every 3 to 4 hours, a 45-to-60-minute delay causes dopamine levels to crash—triggering severe "OFF" rigidity, freezing of gait, tremor, anxiety, swallowing impairment, and preventable falls.
What to verify on your tour:Ask to see how their electronic Medication Administration Record (eMAR) flags time-critical Parkinson’s medications. Ask: "If my parent has doses at 7:00 AM, 10:30 AM, 2:00 PM, 5:30 PM, and 9:00 PM, can your med techs deliver those outside your standard breakfast/dinner med pass?"
#2 • Staff Parkinson’s Training & Drug Safety

Are caregivers trained in Parkinson’s motor fluctuations, and does your consulting pharmacy block contraindicated dopamine-antagonist medications?

Why it matters for Parkinson's:Standard anti-nausea drugs (metoclopramide/Reglan, prochlorperazine/Compazine, promethazine/Phenergan) and standard antipsychotics (haloperidol/Haldol, risperidone, olanzapine) block dopamine receptors and can cause acute motor lock-up, severe confusion, or hospitalization.
What to verify on your tour:Ask whether nursing leadership uses the Parkinson’s Foundation Contraindicated Medications list and whether after-hours on-call providers are alerted before ordering PRN agitation or nausea drugs.
#3 • DBS & Advanced Therapy Experience

Does your nursing team have direct experience caring for residents with Deep Brain Stimulation (DBS) neurostimulators or continuous Duopa/Vyalev pumps?

Why it matters for Parkinson's:Patients with rechargeable DBS generators (Medtronic, Boston Scientific, Abbott) require regular wireless charging checks so the battery never depletes (which causes an acute Parkinsonian crisis). Patients on Duopa (PEG-J) or Vyalev (subcutaneous foscarbidopa/foslevodopa) require daily cassette/syringe and stoma/infusion-site care.
What to verify on your tour:Ask how many current or past residents have had DBS or infusion pumps, and whether staff document daily DBS battery checks on the care plan.
#4 • Handling Freezing of Gait & "OFF" Episodes

How do caregivers respond when a resident has a sudden "OFF" episode or freezes in a doorway?

Why it matters for Parkinson's:A person with Parkinson’s can walk independently to breakfast at 8:00 AM during their "ON" state and be unable to stand from a chair at 10:15 AM during an "OFF" trough. Untrained staff often misinterpret this fluctuation as "behavioral refusal" or pull on the resident’s arms, causing falls.
What to verify on your tour:Look for caregivers trained in external cueing ("Step over the line," rhythmic counting, weight-shifting) and patient standby protocols rather than rushing transfers.
#5 • Fall-Prevention & Physical Design

What Parkinson’s-specific fall-prevention features are built into the apartments, bathrooms, and dining areas?

Why it matters for Parkinson's:Parkinson’s impairs postural reflexes, depth perception, and contrast sensitivity. Patterned carpets, raised doorway thresholds, dim lighting, and tight bathroom turns trigger freezing of gait and backward (retropulsive) falls.
What to verify on your tour:Verify zero-threshold roll-in showers, grab bars on both sides of the toilet and shower, high-contrast toilet seats, smooth non-glare flooring without abrupt carpet-to-tile strips, and motion-activated nightlights from bed to bathroom.
#6 • On-Site PT / OT / Speech Therapy (LSVT)

Are LSVT BIG (physical/occupational therapy) and LSVT LOUD (speech/swallowing therapy) available on-site through visiting Medicare Part B therapists?

Why it matters for Parkinson's:Consistent high-amplitude physical and voice therapy preserves mobility and swallowing safety. Residents in assisted living can receive Medicare Part B outpatient therapy right in their building without exhausting travel.
What to verify on your tour:Ask which home health or outpatient therapy groups visit the campus weekly, whether any visiting therapists hold LSVT BIG / LSVT LOUD or PWR!Moves certifications, and whether the activity calendar includes Parkinson’s-adapted exercise.
#7 • Respite Stays & Trial Moves

Do you offer short-term furnished respite stays (7 to 30 days), and what is the daily respite rate?

Why it matters for Parkinson's:Parkinson’s caregiving is a multi-year marathon. A 2-week respite stay lets a spouse recover from surgery or exhaustion—and gives the family a real-world test of whether the community actually delivers levodopa on time before committing to a permanent move.
What to verify on your tour:Ask for the daily respite rate (typically $200–$325/day in Phoenix Metro), minimum stay days, and whether the pre-admission nursing assessment fee is credited if the resident later moves in permanently.

Why “Call for Pricing” Is the Norm (And Our No-Fabrication Rule): Individual senior living communities rarely publish all-in prices online—"call for pricing" is the industry norm because monthly bills combine a Base Apartment Rent with a tiered Personal Care Fee determined by a pre-admission nursing assessment. OpenPHR never invents a community’s price.

Paying for Parkinson's Senior Living in Arizona • 4 Payer Rules

Does Medicare Pay for Assisted Living? (Plus Arizona ALTCS & VA Aid & Attendance)

#1 Family Misconception — Plain English Answer

Medicare does NOT pay for assisted living or memory care room and board.

This is the #1 financial surprise for Parkinson’s families. Original Medicare (Parts A & B) and Medicare Advantage plans never pay the monthly rent or 24-hour custodial care fees for Independent Living, Assisted Living, or Memory Care.

What Medicare DOES Pay For When You Have Parkinson's:
✓ Short-Term Skilled Nursing Facility (SNF) Rehab (Medicare Part A): Covered ONLY after a qualifying 3-consecutive-day inpatient hospital admission (observation stays do not count under Original Medicare) when daily skilled nursing or PT/OT/Speech rehab is needed. Days 1–20: $0/day coinsurance. Days 21–100: $217.00/day coinsurance in 2026 ($209.50/day in 2025)—covered 100% if the patient has Medigap Plan G or Plan N. Day 101+: Medicare pays $0; patient pays 100%.
✓ On-Site PT, OT, Speech Therapy & Doctor Visits Inside Assisted Living (Medicare Part B): While you pay the assisted living rent privately, Medicare Part B still covers 80% of the Medicare-approved amount (100% with Medigap Plan G) for neurologist visits, LSVT BIG physical/occupational therapy, LSVT LOUD speech therapy, and durable medical equipment (walkers, wheelchairs) delivered inside your assisted living apartment.
✓ Intermittent Skilled Home Health in Independent/Assisted Living (Medicare Part A/B): If a resident living in an independent or assisted living apartment becomes homebound and needs intermittent skilled nursing or therapy ordered by a physician, Medicare covers eligible skilled home health visits at $0 copay.
Arizona Medicaid Long-Term Care (AHCCCS ALTCS)

Arizona Long Term Care System (ALTCS — Administered by AHCCCS)

ALTCS is Arizona’s Medicaid program for seniors (65+) and individuals with disabilities (including Parkinson’s disease) who require a nursing-facility level of care. Unlike Medicare, ALTCS pays for Assisted Living Communities, Memory Care, Adult Care Homes, Skilled Nursing Facilities, AND in-home attendant care across Maricopa County (administered through Mercy Care and Banner – University Family Care ALTCS plans).

January 1, 2026 – December 31, 2026 (2025 shown for comparison)
Gross Income Limit: $2,982 / month gross income in 2026 ($2,901 / month in 2025) — equal to 300% of the Federal Benefit Rate.
Over-Income Miller Trust Path: Over-Income Path (Income-Only / Miller Trust): If a Parkinson’s applicant’s gross monthly Social Security and pension exceed $2,982/month (up to the average monthly private nursing home rate in Arizona), they can still qualify financially for ALTCS by routing income through a statutory Arizona Income-Only Trust (Miller Trust).
Single Countable Asset Limit: $2,000 in countable resources (cash, bank accounts, non-exempt investments).
Married Community Spouse Protection: Community Spouse Resource Deduction (CSRD): When one spouse applies for ALTCS and the other spouse lives in the community, the healthy spouse keeps half of countable joint assets from a minimum of $32,532 up to a maximum of $162,660 in 2026 ($31,584 to $157,920 in 2025), in addition to the applicant’s $2,000.
Exempt Assets: Primary residence in Arizona (up to $752,000 home equity in 2026; $730,000 in 2025—no equity cap if a spouse lives in the home), one vehicle, household goods, and irrevocable burial plans.
Medical PAS Requirement: Must pass the AHCCCS Pre-Admission Screening (PAS) interview demonstrating medical/functional need for nursing-facility level of care (hands-on assistance with ADLs such as transfers, mobility/falls, bathing, toileting, or cognitive supervision for Parkinson’s dementia).
Veterans & Surviving Spouses (VA Benefits)

VA Aid & Attendance Pension + Agent Orange Presumptive Service Connection

Two high-impact VA pathways help Phoenix veterans and surviving spouses pay for Parkinson’s senior living or in-home care:

December 1, 2025 – November 30, 2026 (VA Maximum Annual Pension Rates / MAPR)
Married Veteran
$2,874 / month ($34,488 / year)
Single Veteran
$2,424 / month ($29,093 / year)
Surviving Spouse
$1,558 / month ($18,697 / year)
Net Worth Limit
$163,699 (2026)

Clinical Requirement: Wartime service (90+ days active duty with at least 1 day during a wartime period) + physician certification on VA Form 21-2680 that the claimant requires regular assistance with Activities of Daily Living (bathing, dressing, transferring, feeding) due to Parkinson’s.

🎖️ Agent Orange & Camp Lejeune Presumptive Parkinson’s Rule (38 CFR § 3.309(e)): The VA recognizes Parkinson’s disease (and Parkinsonism) as a presumptive service-connected condition for veterans who served in Vietnam, Thailand, the Korean DMZ, C-123 aircraft, or Camp Lejeune (1953–1987). Qualifying veterans receive tax-free VA Disability Compensation (independent of net worth) and may qualify for VA-paid Contract Nursing Home or Veteran Directed Care in Phoenix through the Carl T. Hayden VA Medical Center.
2. Home Health vs. Home Care • Different Products, Different Payers

Medicare Skilled Home Health ($0 Copay) vs. Private-Duty Home Care ($35–$38/hr in Phoenix)

Families often ask their neurologist for “home health” expecting an 8-hour daily caregiver—or pay $36/hour privately for exercises that Medicare covers at $0. Most Parkinson's households layer both.

DimensionMedicare Skilled Home Health (Clinical / Intermittent)Private-Duty Home Care (Non-Medical / Shift-Based)
Primary PurposeShort-term or intermittent clinical rehabilitation & skilled medical care (e.g., after a fall, hospitalization, medication change, or functional decline).Ongoing daily personal care, fall supervision, and household support so the person with Parkinson’s can live safely at home and the spouse can rest.
Who Comes to the HomeLicensed clinicians: Physical Therapists (PT / LSVT BIG), Occupational Therapists (OT), Speech-Language Pathologists (SLP / LSVT LOUD), Registered Nurses (RN), and Medical Social Workers.Non-medical Caregivers, Home Care Aides, Certified Nursing Assistants (CNAs), or Companions.
What They Do for Parkinson’sGait & freezing-of-gait therapy, transfer training, bathroom safety setup, swallowing/dysphagia therapy, voice projection, orthostatic blood pressure checks, medication regimen teaching.Standby assistance walking to the bathroom, showering, dressing, meal preparation, exact-time levodopa reminders, light housekeeping, transportation, and 4–24 hr respite supervision.
Visit Schedule & DurationIntermittent clinical visits: typically 1 to 3 visits per week per discipline, lasting 45–60 minutes per visit, ordered in 60-day certification episodes.Shift-based blocks: typically 4-hour minimum shifts up to 8, 12, or 24 hours per day, 1 to 7 days per week, for as many months/years as needed.
Who Pays & Patient Cost (Phoenix)Medicare Part A/B ($0 copay for visits under Original Medicare), Medicare Advantage ($0 in-network on most Maricopa County plans; prior auth may apply), or Commercial Insurance.Private Pay ($35–$38/hr median in Phoenix Metro), Arizona ALTCS Medicaid ($0 copay for eligible members), VA Aid & Attendance / Veteran Directed Care, or Long-Term Care Insurance. NOT covered by Medicare.
Plain-English Medical Necessity Checklist • CMS MBPM Ch. 7

Medicare Skilled Home Health Medical Necessity Checklist for Parkinson’s

Policy Source: CMS Medicare Benefit Policy Manual (MBPM) Chapter 7 — Home Health Services & Jimmo v. Sebelius Maintenance Standard (CMS Ch. 7 § 40.2.1) ↗ • View Full Payer Policy Breakdown →

Eligibility Rule: All four CMS criteria below must be documented by the ordering physician (neurologist or PCP) and the home health agency’s OASIS assessment. Under Original Medicare, qualifying patients pay $0 for covered skilled home health visits.

1. Homebound Status (Taxing Effort Standard)

CMS Requirement: The patient must be confined to the home—meaning leaving home requires a "considerable and taxing effort" AND the assistance of another person or mobility device (cane, walker, wheelchair) due to Parkinson’s gait instability, freezing of gait, severe tremor/dyskinesia, orthostatic hypotension, or cognitive impairment.

💡 Parkinson's Documentation Tip:You do NOT have to be bedridden to be "homebound." CMS rules explicitly state that leaving home for medical appointments, adult day health care, religious services, or occasional short family outings (like a haircut or anniversary dinner) does NOT disqualify a Parkinson’s patient.

2. Intermittent Skilled Need (PT, OT, Speech, or Skilled Nursing)

CMS Requirement: The patient requires intermittent skilled Physical Therapy (G0151), Occupational Therapy (G0152), Speech-Language Pathology (G0153), or Skilled Nursing (G0299) that is reasonable and necessary for Parkinson’s disease management.

💡 Parkinson's Documentation Tip:Common qualifying triggers for Parkinson’s: a recent fall or near-fall, new freezing of gait in doorways, difficulty getting out of bed or the shower, quieter voice (hypophonia) or coughing while drinking liquids (dysphagia), or complex new medication adjustments.

3. Jimmo v. Sebelius Skilled Maintenance Protection (No "Improvement" Requirement)

CMS Requirement: Under the federal Jimmo v. Sebelius settlement (CMS MBPM Ch. 7 § 40.2.1), Medicare covers skilled physical, occupational, and speech therapy when the specialized judgment of a licensed therapist is needed to design or safely perform a maintenance program to prevent or slow further deterioration.

💡 Parkinson's Documentation Tip:Because Parkinson’s is progressive, some agencies mistakenly discharge patients claiming they have "plateaued." Have your neurologist write on the referral: "Skilled maintenance PT/OT/SLP medically necessary per CMS MBPM Ch. 7 § 40.2.1 (Jimmo standard) to prevent falls, contractures, and aspiration pneumonia in progressive Parkinson’s disease (ICD-10 G20)."

4. Physician Face-to-Face Encounter & 60-Day Plan of Care (CMS-485)

CMS Requirement: The patient must have a documented face-to-face encounter (in-person or qualifying telehealth) with the certifying physician or NP/PA within 90 days prior to or 30 days after the start of home health care, and the physician must sign the 60-day Plan of Care.

💡 Parkinson's Documentation Tip:Home health is NOT required to follow a hospital stay—your outpatient movement-disorder neurologist or PCP in Phoenix can order Medicare home health directly from a clinic visit whenever your mobility, balance, or swallowing changes.
Parkinson's-Specific Home Setup & Clinical Protocols

4 Things Every Phoenix Parkinson's Family Must Set Up at Home

1. Medication-Timing Supervision & Arizona Scope-of-Practice Rules

In Arizona, non-medical private-duty caregivers can provide exact-time medication reminders, bring water, open containers, and hand pre-sorted pills from a family-prepared or pharmacy-blister-packed organizer so Carbidopa/Levodopa is taken within ±15 minutes. However, non-medical aides cannot fill weekly pillboxes from loose prescription bottles, adjust doses, or program Duopa/Vyalev infusion pumps—those tasks require a family member or a licensed RN/LPN.

2. Mobility, Freezing-of-Gait Cueing & Home Fall Safety

Ask your Medicare home health Physical & Occupational Therapists to perform a room-by-room Parkinson’s fall audit during Visit 1: remove throw rugs, place high-contrast visual cueing tape at narrow doorways and bathroom thresholds where freezing of gait happens, install grab bars and a shower transfer bench, and elevate low sofa cushions to ease sit-to-stand transfers.

3. LSVT BIG & LSVT LOUD Availability in the Home

LSVT BIG (physical/occupational therapy) and LSVT LOUD (speech therapy) require 4 consecutive days per week for 4 weeks (16 sessions). When interviewing a Phoenix home health agency, ask specifically whether they have LSVT-certified clinicians covering your ZIP code—or whether they transition you to an outpatient LSVT clinic once you are no longer homebound.

4. Caregiver Training for "ON/OFF" Fluctuations, Orthostatic Hypotension & Dysphagia

Brief every in-home aide on three Parkinson’s essentials: (1) Wait 60 seconds after the patient sits on the edge of the bed before standing to prevent neurogenic orthostatic hypotension fainting; (2) Sit upright at 90 degrees during meals and for 30 minutes afterward with chin-tuck swallowing techniques if recommended by speech therapy; and (3) Recognize that a blank facial expression (masked facies) or slow verbal response (bradyphrenia) is a Parkinson’s motor symptom, not rudeness or dementia.

11 Questions to Ask Phoenix Home Health & Private Home Care Agencies

Use these questions when interviewing a Medicare-certified home health agency or a private-duty caregiving company in the Valley.

For Medicare-Certified Skilled Home Health Agencies

Do you have Speech-Language Pathologists (SLPs) on staff covering our Phoenix ZIP code, or is speech therapy waitlisted? (Essential for Parkinson’s swallowing/dysphagia and voice therapy).
Are any of your visiting Physical, Occupational, or Speech Therapists certified in LSVT BIG, LSVT LOUD, or PWR!Moves?
How quickly after the neurologist sends the order will a Physical Therapist or RN complete the Start-of-Care (SOC) home visit? (CMS tracks 48-hour timely initiation of care).
Does your clinical supervisor train therapists on the CMS Jimmo v. Sebelius maintenance standard so my family member is not discharged prematurely just because Parkinson’s is progressive?
Will your nurse and therapist reconcile the exact clock times of all Carbidopa/Levodopa doses and check seated-vs-standing blood pressure (orthostatic hypotension) at every visit?

For Private-Duty Non-Medical Home Care Agencies

What specific training do your caregivers receive on Parkinson’s disease (freezing of gait, "ON/OFF" fluctuations, fall standby without pulling on the arms, and dysphagia precautions)?
What is your protocol for guaranteeing that our caregiver prompts and assists with Carbidopa/Levodopa doses within ±15 minutes of the prescribed clock time?
What is your backup staffing protocol if our regular morning caregiver calls out sick at 6:30 AM when my spouse needs help getting out of bed and taking their 7:00 AM levodopa?
What is your minimum shift length (e.g., 4 hours vs. 6 hours), and what are your exact hourly rates for weekdays, weekends, and holidays?
Are your caregivers W-2 employees who are background-checked, bonded, and insured for workers’ compensation (rather than 1099 contractors)?
Are you an approved provider for Arizona ALTCS (Mercy Care / Banner University Family Care) or the VA Homemaker & Home Health Aide program?
